<br />3. It is the employee's responsibility to regain the CDL. Therefore, the employee must-at <br />the employee's expense and on personal time- resolve the issue with the State of <br />Minnesota. <br /> <br />4. Consistent with public service needs and on a case-by-case basis, the city, at its sole <br />discretion, may allow an employee who has a work permit but no CDL to work in a light <br />duty capacity for up to one year. <br /> <br />If the employee's CDL privileges are restored within one year, the employee may return <br />to work after proof that the CDL privileges have been reinstated has been submitted to <br />the department head and human resources manager. If the employee's CDL privileges <br />are not restored within one year, discipline may be imposed up to and including <br />termination. <br /> <br />Employees who lose their CDL twice in a five year period may be disciplined for the <br />second offense, up to and including termination. <br /> <br />Damage to VehicleslEquipment <br /> <br />1. Every employee shall notify the department head as soon as possible of <br />any damage to a city vehicle, another vehicle and/or personal injury, when <br />the employee has been driving the vehicle or when the vehicle has been <br />in the employee's possession or control. <br /> <br />2. The department head shall report the incident to the Deputy City Administrator and the <br />deputy city administrator or a designee will complete a vehicle claim form for insurance <br />purposes. In addition, the employee shall complete a report with the police department in <br />the jurisdiction where the incident occurred. <br /> <br />3. In the event of personal injury to the employee, First Report ofInjury forms can be found <br />on the city's intranet. <br />
