<br />3. Cost of Services and Billing Methods: <br /> <br />Compensation and payment options for building inspection and related services:. <br />a) A set hourly rate of $25.00, to include time for administration, meetings, plan <br />review, inspection scheduling, field inspections and code enforcement; <br />b) Consultant will submit bi-monthly billing invoices to the City clerk for payment of <br />services rendered on a bi-monthly basis. <br /> <br />4. Insurance: <br /> <br />All parties shall maintain liability coverage protecting itself and employees against any <br />usual claims in amounts that comply with State statutory requirements. Policies shall be <br />in effect during the terms of the contract agreement. See City of Ramsey RFP for <br />Proposal details, issue date October 16,2009. <br /> <br />5. Terms of Contract and Termination: <br /> <br />The contract terms remain in effect on a semi-annual or six (6) month calendar bases. <br />The contract may be terminated by either party with a 45 day advance written notification <br />of termination mailed via the U.S Postal Service. The initial contract start date will <br />depend on documentation proof of compliance with contract terms and the City Council . <br />approval of a contract services agreement with the consultant Building Permit & <br />Inspection Services, LLC. <br /> <br />6. Building Inspector & Zoning Inspector Qualifications: <br /> <br />Steven Horsman, Chief Operating Officer, Building Permit & Inspection Services, LLC: <br />. Certified Building Inspector, International Conference of Building Officials, <br />Certificate # 1086474-10, issued in 1997, expiration date January 2011; <br />. Certified Building Official, State of Minnesota, Certificate # MN-1981, issued in <br />1998, expiration date November 2010; <br />. Building Inspection Technology Certificate, Inver Hills Community College, MN, <br />issued December 2006; <br />. MNIMS Incident Management Training Certificate, Scott County Emergency <br />Management Division, issued September 2006. <br />. Zoning Administration Certificate, University of Wisconsin-Madison, issued 1999. <br /> <br />7. Contract Service References: <br /> <br />Mr. Mark Nagel, Asst.
