REFERRALS <br /> <br />o' All referrals must come fi.om a registered agency/organization. <br />· All clients must fall within one of the categories to qualify for assistance (see list for criteria). <br />· Ail forms must be filled out completely by the client or referring agency/organization prior to <br /> appointment. <br />· Agency/organization must complete back side of£ntake Form for purpose of verification. <br />· Separate applications cannot be made for individuals living in. the same household. <br />· All clients must be Anoka County residents. <br />· Referrals are valid for three months from date issued. <br />· Items are given to families .at no cost, however, donations are welcomed and appreciated <br /> <br />APPOINTMENTS <br /> <br />· An appointment must be made before coming to the warehouse. Call 7834735 Monday-Friday (8- <br /> 4:30) to schedule an appointment. <br />· All clients must have their Intake FOrm and Item Request IN HAND when making an appointment. <br />· The client whose name appears on the Intake Form must be present for the appointment; photo <br /> identification will be required. <br />· Clients MUST bring their completed Intake Form & Item Request Form with them to the warehouse <br /> for their appointment or they will not be served. <br />· Clients are limited to one furniture appointment in a year. Clients may receive up to four major <br /> furniture items (not including beds) at that time. ACCAP reserves the right to extend additional items <br /> in cases of extenuating circumstances. <br />· Clients or referring agency/organization must cancel an appointment at least 24 hoursin advance if <br /> client cannot keep an appointment. Appointments not cancelled in advance are considered a "no <br /> show". "No show's" must wait 14 days to reschedule. <br /> <br /> WAREHOUSE <br /> <br />· Upon arrival to the warehouse, Staffwill review 'completed forms and photo identification. <br />-o A client may have. a spouse, caseworker, fi/end, or interpreter (only one) accompany them through the <br /> warehouse (no children, please) <br />· Dressers, kitchen tables, and other items are not always available. The Home to Home Program may <br /> place limits on those items that' are in 'short supply so that more families may benefit from those <br /> donations. <br />· Clients should come prepared (a track, trailer,, etc.) to take the items at the time of their appointment. <br /> You must bring your own help to load.your vehicle and your own rope to tie down the. items. <br />· Abusive behavior, inappropriate language, or fa/lure to follow guidelines will not betolerated. I/this <br /> occurs, the client will be asked to leave and the referring agency/organization will be notified. The <br /> Home to Home Program reserves the right to revoke any referral or to deny service to clients that do <br /> not comply with the program rules. If necessary, police will be called. <br /> <br /> <br />
