I <br />I <br />I <br />I <br />I <br /> <br />i <br />I <br />I <br /> APPROVED: <br /> <br /> ,aRAb, <br /> <br /> 3rdina: <br /> <br />! <br />I By: <br /> <br />i Title: <br /> Date: <br /> <br />I By: <br />I Title: <br /> <br /> Date: <br />I <br />I <br />I <br /> <br />THIS GRANT AMENDMENT Number 2 for Grant Agreement Number 1 [G00021605, shall be interpreted pursuant to the laws <br />of the State o[ Minnesota, between the State of Minnesota, acting through its Minnesota Department of Health (hereinafter <br />STATE) and Ramsey Police Department, an independent organization, not an employee of the State of Minnesota, address: 15153 <br />Nowthen Boulevard Northwest, Ramsey, Minnesota 55303 (hereinafter" GRANTEE"), witnesseth that: <br /> <br />WI-IEREAS, the STATE, pursuant to IVlinnesota Statutes Section 144.05, is empowered to plan, facilitate, coordinate, provide and <br />support the organization of services for the prevention and control of illness and disease, and the limitation of disabilities resulting <br />therefrom, <br /> <br />WH:EREAS, the STATE, pursuant to Minn. Stat. ff 144.396 is empowered to award competitive grants to eligible applicants for <br />projects and initiatives directed at the prevention of tobacco use by Minnesota youth, <br /> <br />WHEREAS, GRANTEE represents that it is duly qualified and willing to perform the services set forth herein. <br /> <br />NOW, THEREFORE, it is agreed: <br /> <br />TERMS OF AGREEMENT This grant agreement shall be effective on January 9, 2001, or upon the date that the final <br />required signature is obtained by the STATE, pursuant to Minn. Stat. § 16C.05, Subd. 2, whichever occurs later, <br />and shall remain in effect until SeptemL,er 30 December 31, 2001, or until all obligations set forth in this grant agreement <br />have been satisfactorily fulfilled, whichever occurs first~ GRANTEE understands that NO work should begin under this <br />grant agreement until ALL i'equired signatures have been obtained, and GRANTEE is notified to begin work by the <br />STATE's Authorized Representative. <br /> <br />IN WITNESS wHEREoF, the parties have caused this grant agreement to be duly executed intending to be bound thereby. <br /> <br />GRANTEE certifies that the appropriate person(s) have <br />executed the agreement on behalf of the GRANTEE as <br />required by applicable articles, by-laws, resolutions, or <br />ordinances. <br /> <br />2. STATE AGENCY: <br />Grant Agreement approval and-certification that STATE funds <br />have been encumbered as required by Minn.
