I By: i Title: Date: I By: I Title: Date: I I I THIS GRANT AMENDMENT Number 2 for Grant Agreement Number 1 [G00021605, shall be interpreted pursuant to the laws of the State o[ Minnesota, between the State of Minnesota, acting through its Minnesota Department of Health (hereinafter STATE) and Ramsey Police Department, an independent organization, not an employee of the State of Minnesota, address: 15153 Nowthen Boulevard Northwest, Ramsey, Minnesota 55303 (hereinafter" GRANTEE"), witnesseth that: WI-IEREAS, the STATE, pursuant to IVlinnesota Statutes Section 144.05, is empowered to plan, facilitate, coordinate, provide and support the organization of services for the prevention and control of illness and disease, and the limitation of disabilities resulting therefrom, WH:EREAS, the STATE, pursuant to Minn. Stat. ff 144.396 is empowered to award competitive grants to eligible applicants for projects and initiatives directed at the prevention of tobacco use by Minnesota youth, WHEREAS, GRANTEE represents that it is duly qualified and willing to perform the services set forth herein. NOW, THEREFORE, it is agreed: TERMS OF AGREEMENT This grant agreement shall be effective on January 9, 2001, or upon the date that the final required signature is obtained by the STATE, pursuant to Minn. Stat. § 16C.05, Subd. 2, whichever occurs later, and shall remain in effect until SeptemL,er 30 December 31, 2001, or until all obligations set forth in this grant agreement have been satisfactorily fulfilled, whichever occurs first~ GRANTEE understands that NO work should begin under this grant agreement until ALL i'equired signatures have been obtained, and GRANTEE is notified to begin work by the STATE's Authorized Representative. IN WITNESS wHEREoF, the parties have caused this grant agreement to be duly executed intending to be bound thereby. GRANTEE certifies that the appropriate person(s) have executed the agreement on behalf of the GRANTEE as required by applicable articles, by-laws, resolutions, or ordinances. 2.
