CDBG FUNDING REQUEST APPLICATION <br /> <br />I. GENERa~L INFORMATION <br /> A. Proposed Activity <br />Arc is requesting $1,000 from the City of Ramsey CDBG funds. <br />Funds will be used to support the most essential services offered <br />to residents of Anoka County and specifically the City of Ramsey. <br /> <br />These services encompass a wide range of activities including <br />leisure services, information and referral services; one-to-one <br />advocacy and problem solvin~ with adults with disabilities and <br />the Toy :,nd Adaptive Equipment Lending Library Arc has over 650 <br />members in Anoka C~unty and provides direct services to an <br />estimateU 900 to P,000 peop~.e in the county. ~rc provides <br />information and di~3ect serve to apprc~imately 50 people from the <br />City ot K~rasey. <br /> <br />Arc has L_ovided these serviues to Anoka County and the City of <br />Ramsey r~idents t~r over te~ years a~.d would w=icome th~- <br />continued support of the Cindy through CDBG funding. <br /> <br />Cor~munit,z !!e. eds A(,~ressed by this Project: <br /> <br />Adult se..~iges: Over the years, sez~ices for · 4ults witl~ <br />developm~a! disabilities b~ve focused on assi3ting people to be <br />product~e, participating ~ncmbers of aheir horn'~ communitzes. <br />Emphasis hns shifted from l~rge institutional ~esidenti~k <br />services to typicam community homes serving fouz to six people, <br />and supp~ted employment in typical jobs rather than large <br />sheltered workshops serving only people with disabilities. This <br />change bas proven to be successful - adults wi~h mild to moderate <br />disabilities are participating in their communities and working <br />in jobs that contribute to the community and adults with more <br />severe ,'iisabilities are finding opportunities to be more <br />involved. <br /> <br />Arc has been instrumental in supporting legislation to make these <br />changes possible and providing one on one assistance to adults <br />with disabilities and their families to access the services <br />available. Arc provides information and support to adults living <br />in group homes and individuals living in your city. For these <br />individuals, community participation is a personal goal and also <br />a challenge given the limits that a developmental disability <br />places on a person. <br /> <br />Through the Arc individual advocacy program, People First Group <br />and information and referral services Arc provides one on one <br />support to adults with developmental disabilities to help: <br />identify supports and services to live successfully in the <br />community; apply for services and complete intake processes; <br />resolve problems and issues with services; identify and access <br />social/leisure opportunities; and increase leadership skills and <br />involvement in the community. <br /> <br />Typical services provided by Arc include: helping a family find <br /> <br /> <br />
