We are located <br />one half mi!~ west of the North Town Mall. We provide individual, couple, family <br />and adolescent therapy, as well as psychiatric s~rvices. Weprovide these services <br />go person~ living in Anoka County who do not have insurance coverage. We have a <br />sliding fe~ ~cale progr~'~ based on a persons ability to pay -- we do not turn <br />away client.~ due to an ±~ability to pay. Approximately 45% of our clients are · <br />eligilbe fo.~ the sliding fee program. 85% of th~ s~iding fee scale clients fall <br />u~der the c~tagory of lo~-i~come with an annual imcome of less that $22,0C0. We <br />did not track City of Ra~asey indivi~'als/familic~ last year ~ we were nog <br />obligated t~ do so by a Zunding program. With A~,~ka County growing to thc north- <br />west, we do ~nti¢ipate ~bat residem~s from Ramsey will only iz~crease as wakl. <br />fhese low-~,~come client~ benefit from mental heai~h services in many way~ therapy <br />can posi~i~ ~ly impact ~ ~ environment in the hom~ marital satisfaction, ~arenting, <br />employmen~ amd employab~ity, physic~i health, chemical healt~ and overal? citizenry~ <br />Without p'~cram funding our clinic ~,~ould not be able to provide the services that <br />we do, whi~.~ allows ind~iduals and '~heir families to recieve the 'least restrictive <br />type of in~z~vention a~ avoid hospiualizationo <br /> <br />P~OJECT DU~iTION: Our ~.~aed for fum~ing is ongoing, as we are a non-profiu organi- <br />zation. W~a ~.o intend t~~ Oe in the con,unity as long as funding and need are available° <br /> <br />VI. Su~=zize fundins~ Enlosed is our 1999 budoet which describes revenue sources° <br /> We a~e asking the'City of Ramsey to fund our program with $1,500.00. We do <br /> rec~eve CDBG funding from the City of Blai~e $5,000., t~e City of Andover $1,0C~o <br /> the City of Anoka $2,000. and Coon Rapids $24,200.00. Anoka County subsidizes <br /> out sliding fee program (mentgl health/psychiatry) with $103,000. We are also <br /> providers for several major insurance companies (HM0~s) and do accept Medical <br /> Assistance and Minnesota Care. <br /> <br />V. Our full fee for sez~zice is 80.00 for psychotherapy and $145.00 for psychiatry° <br /> Reimbursement' rates are substantially lower and only 3% of our clientel pay <br /> the full fee for services. The average reimbursement rate from an insurance <br /> company is $55.00 and the agency must write off the rest. Our sliding fee <br /> program is based on household income and the number of dependents. The clinic <br /> discounts that program byl0% so the full fee is $72.00 per session. <br /> <br />IX.~ North Suburban Counseling Center is not part of a larger effort. There are <br /> two other non-profit mental health centers in Anoka County and we do refer <br /> back and forth according to client need. One clinic primarily sees children, <br /> while North Suburban is general out-patient. <br /> <br />X. CONTACT: Gretchen Wesche-Sherman,LICSW -- Clinic Director.
