I <br /> <br />Contract #00-2216 <br /> <br />RECORDS Aa'rD MONITORING <br /> <br />CONTRACTOR will maintain records that reflect ali revenue received and alt direct and indirect <br />costu incurred in performing Purchased Services. <br /> <br />CONTRACTOR will assist the COUNTY with documentation necessary to complete forms and <br />reports required by State and Federal agencies. <br /> <br />All reports requested by the COUNTY must be received by the COUNTY within 30 days following <br />the request, unless a time extension is approved by the COUNTY. <br /> <br />The COUNTY and ACCFC has the right tO monitor and evaluate CONTRACTOR's performance <br />of Purchased Services. <br /> <br />On a monthly basis, CONTi~'CTOR agrees to g/ye ACCFC a completed report using the <br />Elementary School Law Enforcement Project form. <br /> <br />On a quarterly basis, CONT1L¢CTOR agrees to give ACCFC summary assessment data on cases in <br />which intervention services ended during the calendar quarter. <br /> <br />Twice a year, CONTRACTOR agrees to ~Ve Parent Surveys to those parents that it has contact with <br />during the selected survey months. <br /> <br />CONTRACTOR agrees that its bonds, records, documents, accounting procedures andpractices, and <br />other papers relevant to the Contract are .subject to examination, duplication, transCription, and audit <br />by Anoka County, Minnesota Department of Human Services [DHS], Le~slative or State Auditor <br />under IVfinn. Stat. § 16C.05, subd. 5, and U.S. Department of Health and Human Services and the <br />U.S, Comptroller General, or a duly author/zed representative. <br /> <br />6.8. t Contractor agrees to maintain the documents for 6 years from the last date services were' <br /> provided or payment made. <br /> <br />NONDISCRIMINATION <br /> <br />Anoka County is an Affirmative Action/Equal Opportunity Employer.
