I <br />il <br /> <br /> I <br /> I <br /> I <br /> I <br /> I <br /> I <br /> I <br /> I <br /> I <br /> I <br />I <br />I <br />I <br />I <br />I <br />I <br /> <br />ANOKA COUNTY HIGHWAY DEPARTMENT <br /> <br /> GUIDELINES FOR UTILITY PERMIT APPLICATIONS <br /> <br />General Information: <br />This permit is used for the installation or repair of public utilities such as gas, telephone and electric <br />within county dght of way. Installation and repair of city owned utilities such as sewer and water mains <br />also requires a permit. One application is required for each county highway upon which work is planned. <br />Large projects, such as installation of transmission lines, which involve more than one county highway <br />will require separate permit applications for each highway involved. Multiple work sites along one county <br />highway may be included on one permit provided they am similar in nature or relate to the same projecL <br />If multiple work sites am listed on one application, final inspection by ACriD will be carried out upon <br />completion of work at all sites listed. <br />NOTE: Listing non-related multiple work sites several miles apart on the same highway is not <br /> acceptable. <br /> <br />Each application is to be submitted with two work site drawings. Drawings am to include placement of <br />utility or work site in relation to: <br /> · Centedine Of Highway <br /> · Right Of Way/Property Boundaries <br /> · Edge Of Mat/Curb <br />NOTE: Applications mcaived without drawings will be mtumed to the applicant without approval. <br /> <br />Verbal permission will only be given for emergencies or hardship cases, not for convenience. All verbals <br />are to be followed up with a written application. <br /> <br />Placement of any above ground structure is to be beyond the clear zone unless a break-away type <br />design is used. If existing R.O.W. is inadequate, structures am to be placed as far away from centerline <br />as possible and should be break-away type design. <br /> <br />Pneuma-gopher operations.are to fully comply with the special conditions listed under 'Horizontal Boring <br />And Jacking' on the bacl(side of Permit. <br /> <br />"Windows" opened to expose existing utilities are to be restored same day. <br /> <br />Traffic Control is the responsibility of the applicant.
