We are asking for a grant of $3,000 to be used to purchase
capital equipment and other items needed for the start-up of the program: we anticipate total
expenses will come to $146,250.

Anoka CIP is a Day Training and Habilitation Program (DTH) developed to meet the needs of
graduates from the Anoka-Hennepin School District and other individuals with severe
disabilities. This program will provide job training and placement, therapies when needed,
vocational support services and will increase self-esteem and relationships with peers.

Anoka CIP was formed because of an exciting collaboration between the City ofAnoka, Anoka
County, the Anoka Enterprise Park businesses, and Rise. The partnership occurred because of a
need to provide services to Anoka residents with specialized needs. Since existing programs were
not able to accommodate these individuals, the county asked Rise to develop a program for them.
And the City of Anoka helped make this idea a reality by approving the location of the program
in the park. The 30 businesses in Enterprise Park were very supportive of Rise's presence also.

The development of Anoka CIP through this unique effort between government and the non-
profit and for-profit sectors demonstrates strong community involvement. It reflects a
community's interest in helping people with disabilitie~ become valued and productive citizens
regar, dless of the severity of their disabilities. Rise is pleased with its long-term presence in the
Anoka community and the opportunity to provide services to its residents.

Construction of Rise's 13,000 square foot building began in October and completion of it is
expected in late January or early February. Initially, 20 people will be served and an additional 25
persons will be added over several years. A CDBG from the City of Ramsey will help with the
early development of the program.

Committed to integration, employment, attd bzdependence for all people
through service excellence.
