QUESTIONNAIRE <br /> CDBG-FUNDED ACTIVITIES <br />COMPLIANCE WITH REHABILITATION ACT OF 1973 (SECTION 504) <br /> <br />As you answer this questionnaire, consider potential clients with <br /> - hearing impairment -vision impairment <br /> - mobility impairment - mental handicaps <br /> - conditions which keep them home- or care facility-bound. <br /> <br />What services are provided? Family Life Mental Health Center provides professional <br />based mental health services (counseling and medical psychiatric care) to <br />low income and disable persons and families. Services include: medical <br />management of chronic mental illness, day treatment to enhance quality of <br />life for those with a disabling mental illness and anger management group <br />for domestic assault perpetrators. <br /> <br />Where do you provide services? <br /> <br />1428 Fifth Avenue South <br />Anoka, MN 55303 <br /> <br />Indicate whether or not each physical location is accessible to those with mobility <br />impairments, Facility is completely handicapped accessible. Many patients are <br />wheel chair bound, have sight and/or hearing handicaps. <br /> <br />If any of your sites are inaccessible, what accommodations can you make to offer <br />services to those who cannot access your place of business? <br /> <br />5: <br /> <br />How do you communicate notices regarding your services or employment? How will you <br />communicate with those with hearing or vision impairments? <br /> <br />Family Life Mental Health Center is well known in the Anoka area having <br />served the low income disabled population since 1978.
