V

VI

VII

VIII

cj

Services: Our services primarily deal with mental health services to the
chronically mentally ill, keeping them out of hospitalization, away from be-
coming community problems requiring the services of police in emergency
situations, assisting the overall safety and liability of the community and
assisting those who are hurting because of chronic depression and/or other
illnesses. Because of inability to perform at normal levels on a job most are
jobless and dependent on welfare or social agencies often for their very
existence.

Summary of Funding: We are requesting a total of $5,000 from the City
of Ramsey. We will be requesting CDBG monies from other communities and
are asking for donations in the next 1-1/2 years for replacement of furnishings
but also to subsidize care for those who cannot otherwise receive services.

Is Project Part of a Larger Effort? The project is essential and in part
to maintain the survival of Family Life Mental Health Center in an era of
managed care, reduced welfare benefits and the lack of insurance for mental
illness and social programs to assist those who cannot fully help themselves.

Project Contact Person: The primary contact person is Robert Sullivan,
Director of Clinical Services. Billing and financial contact is Jean Brownell.
