COMPLIANCE WITH <br /> <br />QUESTIONNAIRE <br />CDBG-FUNDED ACTIVITIES <br /> REHABILITATION ACT OF 1973 <br /> <br />(SECTION 504) <br /> <br />As you answer this questionnaire, consider potential clients with <br /> - hearing impairment . vision impairment <br /> - mobility impairment - mental handicaps <br /> - conditions which keep them home- or care facifity-bound. <br /> <br /> What services are provided? <br /> <br />Alexandra House, Inc. is a non profit, tax exempt, domestic violence service provider for <br />women and families in Anoka County and surrounding communities. Services are free <br />and intrude support, advocacy, intervention, professional training and community <br />education provided by staff, interns and volunteers. <br /> <br />2. Where do you provide services? <br /> <br />In addition to a 24 hour emergency shelter located in Blaine, Alexandra House has <br />offices in the Anoka County Human Services building, the Anoka County Courthouse <br />and at Unity and Mercy Hospitals. <br /> <br />Indicate whether or not each physical location is accessible to those with mobility <br />impairments. <br /> <br />All locations are accessible to those with mobility impairments. <br /> <br />tf any of your sites are inaccessible, what accommodations can you make to offer <br />services to those who cannot access your place of business? <br /> <br />NA <br /> <br />How do you communicate notices regarding your services or employment? How will you <br />communicate with those with hearing or vision impairr:nents? <br /> <br />Alexandra House ha:. a TTY phone device for those who are heating impaired and <br />routinely uses the services of a sign language interpreter. In addition, our Executive <br />Director is fluent in signing capability. Accommodations and assistance are offered as <br />well to those who have vision impairments. <br /> <br />In addition, Alexandra House publicizes its services and employment opportunities in <br />newspapers, many of which are available as Dial in News over the phone for vision <br />impaired.
