QUESTIONNAIRE <br /> CDBG-FUNDED ACTIVITIES <br />COMPLIANCE WITH REHABILITATION ACT OF 1973 (SECTION 504) <br /> <br />As you answer this questionnaire, consider potential clients with <br /> - hearing impairment - vision impairment <br /> - mobility impairment - mental handicaps <br /> - conditions which keep them home- or care facility-bound. <br /> <br />What se~ices are provided? <br /> <br />Individual advocacy, information and referral services, <br />recreation activities, workshops and support groups are <br />available to people challenged by these disabilities. <br /> <br />Where do you provide services? <br />Services are provided in the Arc office, <br />homes, and in community resources. <br /> <br />in oarticipants <br /> <br />Indicate whether or not each physical location is accessible to those with mobility <br />impairments. <br /> <br />Services provided within the Arc office are accessible. <br />Prior to scheduling use of community resources, a screening <br />for accessibility occurs. <br /> <br />I1 any of your sites are inaccessible, what accommodations can you make to offer <br />services to those who cannot access your place of business? <br />Not applicable. <br /> <br />Howdoyoucommunicatenoticesregardingyourse~icesoremployment?
