CITY OF RAMSEY <br />COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) <br /> FUNDING REQUEST FORM <br /> PROJECT YEAR 1997-1998 <br /> <br />NAME OF ORGANIZATION Anoka County Community Action Program, Inc. <br /> <br />I. PROJECT NAME: Ramsey Senior Outreach Program <br /> <br />II. PROJECT ACTIVITY: <br /> <br />A. Briefly describe the project. <br /> <br />1. The project location will be the City of Ramsey. <br /> <br />2. The project activity consists of a Senior Outreach <br /> Worker identifying and meeting with vulnerable and <br /> isolated seniors and disabled adults, and assisting <br /> them in obtaining financial, social, medical and <br /> nutritional services. These may include food <br /> programs, Public Health Nursing, chore services, <br /> Medical Assistance, crisis home repair, and others. <br /> The Outreach Worker also works with community groups, <br /> senior centers, city staff and other service providers <br /> to identify appropriate clients and to determine the <br /> most appropriate resources available to them. <br /> Frequently, the Outreach Worker is contacted by a <br /> community group or a business interested in providing <br /> assistance to those in need, and requests help from <br /> the Outreach Worker in identifying such residents who <br /> would benefit from such programs as Christmas baskets <br /> or house painting. <br /> <br />Finally, the Outreach Worker meets every two weeks <br />with the four other Outreach Workers in Anoka County <br />and the Coordinator of Senior Programs. This meeting <br />provides support, education, information and resource <br />development for the staff. The Outreach Worker also <br />receives daily supervision and guidance from the <br />Coordinator of Senior Programs at ACCAP. <br /> <br />ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC. <br /> <br /> <br />
