QUESTIONNAIRE <br /> CDBG-FUNDED ACTIVITIES <br />COMPLIANCE WITH REHABILITATION ACT OF 1973 (SECTION 504) <br /> <br />As you answer this questionnaire, consider potential clients with <br /> - hearing impairment -vision impairment <br /> - mobility impairment - mental handicaps <br /> - conditions which keep them home- or care facility-bound. <br /> <br />1. What services are provided? <br /> <br />Senior Outreach services are provided to iow-income seniors and disabled <br />adults in the community. Outreach Workers provide case management, information <br />and referral services, as well as advocacy services to clients 60 and older <br />or those with a disability. <br /> <br />Where do you provide services? <br /> <br /> Outreach Worker meets with clients in their own places of residence. The <br /> program is administered through Anoka County Community Action Program, Inc., <br /> which is located in the Human Services Center of Anoka County, 1201 89th <br /> Avenue NE, Blaine, MN 55434. Outreach Workers also meet with service <br /> providers at the providers location either with the client or alone, on client's <br /> behalf. <br />Indicate whether or not each physical location is accessible to those with mobility <br />impairments. <br /> <br />The Human Service Center of Anoka County is completely accessible to those <br />with mobility, vision, and hearing impairments. It was built in 1991, and <br />exceeds federal standards. Clients' homes.may or may not be accessible. Other se <br />service providers may or may not be accessible. <br /> <br />If any of your sites are inaccessible, what accommodations can you make to offer <br />services to those who cannot access your place of business? <br /> <br />How do you communicate notices regarding your services or employment? How will you <br />communicate wi'th those with hearing or vision impairments? <br /> <br />Senior Outreach services are advertised through flyers, community presentations, <br />word-of-mouth, announcements at senior centers and city council meetings, <br />and individual promotion by the Outreach Worker. Employment notices are <br />posted at ACCAP, are advertised in local papers, and are distributed to the <br />State Job Service, which has special accomodations for those with vision, <br />language, hearing, or mobility impairments. <br /> <br />CDBG.55 <br /> 3,r25.'9 ~ <br /> <br /> <br />
