PRELIMINARY PROJECT REQUEST FORM <br /> <br />I. Opportunity Services - New Facility <br /> <br />II. Project is located at 2383 Coon Rapids Blvd. <br /> <br />A new facility will be constructed to ~rovide <br />services to persons with disabilities in Anoka <br />County. This facility will be designed to be able <br />to serve as the work service for up to 150 persons. <br />All persons served are low income. <br /> <br />III. This is a one year project. <br /> <br />IV.
