The only city which has an.y written policy is Blaine. <br />Most other cities simply put a sign up when requested. Several <br />cities did mention that they keep a record of the child's name, <br />address, and age. <br /> <br />Woodbury, in padiculai', requires a written request for such a sign. <br />In addition, the sign file is reviewed yearly to ensure that no family <br />has moved or' become of such age that the sign is'no longer <br />necessary. Woodbury did decline a request for a blind child'sign <br />since there are no provisions for such. in the ADA. <br /> <br />I suggest we draft a request form which contains all the necessary <br />information and delineates the child's age at which the sign will be <br />erected ~nd dismantled. <br /> <br /> <br />
