Excavation Permit Fee: $ [To be filled out by the City.] a: City Cost: $ b: Cost of Restoration (if done by the City) $ c: Degradation Cost: $ (In an amount to fully cover costs associated with the decrease in the useful life of the Right-of-Way caused by an excavation.) 2. Obstruction Permit Fee: $ (In an amount sufficient to cover the City's Cost.) Disruption Fee: $ (Note: The City will impose $ per day for delays in the completion of an Excavation or Obstruction beyond five (5) days past the period indicated in this Permit,)' Note I: "City Cost" means the actual costs incurred by the City for managing Rights- of-Way including, but not limited to costs associated with registering of applicants; issuing, processing, and verifying Right-of-Way Permit applications; revoking Right-of-Way Permits; inspecting job sites; creating and updating mapping systems; detemfining the adequacy of Right-of-Way restoration; restoring work inadequately performed; maintaining, supporting, protecting, or moving user equipment during Right-of-Way work; budget analysis; record keeping; legal assistance; systems analysis; and performing all of the other tasks required by this Chapter, including other costs the City may incur in managing the provisions of this Chapter except as expressly prohibited by law. Note 2: The Excavation or Obstruction Permit Fees must be paid at the time of application or within thirty (30) days of billing therefore by the City. In no event will the City issue a Permit prior to payment of Permit Fees.
