The only city which has ~n.y writ-ten policy is Blain~.. <br />Most other cities simply put at sign up when requested. Several <br />cities did mention that they keep a record of the child's name, <br />address, ~nd age. <br /> <br />Woodbury, in padiculai', requires a written request for such a. sign. <br />In addition, the sign fire is reviewed yearly to ensure that no family <br />has moved or' become of such age that the sign is'no longer <br />necessary. Woodbury did decline a. request for a blind child'sign <br />since there a~re no provisions for such. in the ADA. <br /> <br />I suggest we draft a request form which contains all the necessa, ry <br />information ~nd delineates the child's age at which the sign will be <br />erected stnd dismantled. <br /> <br /> <br />
