This permit is based on conduct of operations as described in the Permit Holders submittals of February 22, 1991 as follows: History of Business Operational Plan Site Plan (not dated) and as amended by the revised site plan submitted to the City Council 4/23/91 and the Site Plan for building expansion dated February 2, 1996. The identified supporting documents are incorporated herein by reference. performance Review. The performance of this operation shall be reviewed by the City Council to identify problem areas, concerns, and to evaluate whether conditions of this CUP need to be modified or procedural changes made. An initial performance review shall occur within six months after commencement of operations. The Permit Holder shall be required to provide to the City, on an annual basis, documentation to support that all licensing and permitting requirements of the regulatory agencies are being complied with. The motion for the adoption of the foregoing resolution was duly seconded by Councilmember Beyer and upon vote being taken thereon, the following voted in favor thereof: Mayor Glen Hardin Councilmember Kenneth Peterson Councilmember Gerald Zimmerman Councilmember Sheila Beyer Councilmember Carolyn Beahen and the following voted against the same: None and the following were absent: None ... and the following abstained: None whereupon said resolution was declared duly passed and adopted by the Ramsey City Council this the 9th day of April, 1996. Alloy Recovery Company hereby acknowledges receipt of this permit and that it has reviewed the conditions of this permit and has agreed that it will comply with the terms of this Permit.
