Page Two

N~m~ and Address of AgeDcY:

Community Emergency Assistance Program, Inc.
1209 89th Ave. NE, Suite 130,
Blaine, MN 55434

Administrative Address:

Kent P. Campbell, Director of Organizational Development
CEAP
7251 Brooklyn Blvd
Brooklyn Center, MN 55429
566-9600

year of Incorporation:

CEAP was incorporated on May 1, 1971. The CEAP branch in
Anoka County started in operation on July 13, 1987.

Goals and Principal Objectives of Organization:

The purpose of CEAP is to prevent a crisis for an individual
or family through the provision of intervention services,
and to assist clients in receiving long-term self
-sufficiency services. Mission Statement: CEAP is
available to the people of the community to answer their
critical needs while preserving their dignity as human
beings.

Amount Requested:

CEAP is requesting $2,000 from CDBG funds.

CDBG funds will be used for direct service needs such as
Intervention, Family Services and the MAC program. Wi=h the
growing number of people in our communities having
difficulty making ends meet, CEAP's services have continued
to expand.

100% of the funds will be used for Ramsey residents for
direct services.

Rationale for Request:

There is evidence that relocation from Coon Rapids to Blaine
(in the Anoka County Human Service Building) has produced
greater awareness and subsequently greater usage of CEAP
emergency services.
