Briefly describe the project <br /> <br />1.~ Project Location: Rise has facilities/locations in <br />.Spring Lak~ Park, Coon Rapids, Brooklyn Center, ~ortheast <br />Minneapolis~ downtown St. Paul, and Center City. Rise serves <br />individuals~from a wide qeoqraphic area in the metropolitan and <br />surroundinq~communities <br /> ~ · <br /> <br />2. Project Activities: Rise provides job traininq and <br />employment OPportunities for individuals with mental, physical, and <br />developmental .disabilities. Many program participants work in <br /> r , <br />.community-b~sed qobs. Most cf them do not drive and therefore, <br />depend on e%ther qeneric kinds of transportation such as the MTC, <br />Metro Mobility, Anoka Traveler, and car pools, or Rise <br />transporter%oD.. Rise only transports those who cannot access public <br />transporter%on.or have no other options. Sixteen vans are used <br />daily. Thre~ have over !80,000 miles on them and need to be <br />replaced. R~se proposes to purchase a 15 Dassenqer van to replace <br />one of its ~aq~nC vehicles so it can continue to transport Rise <br />pro,ram Dar~icip~nts~ . efficiently· and safely. <br /> <br /> 3..Detail the number of persons.benefitting: In 1992 Rise <br />provided j~. ~rainin~, employment, t~ansitional housinG, and <br />supported !~¥iDc se_~vices to 1187 persons with disabilities. <br />Recently Ris~ ex~Derienced an increased demand for its procrams and <br />the need t~ serve people with more severe disabilities. Rise <br /> <br />pro4ects th~ averace daily census to increase this year to 15 <br />percent an~_ total apDroximate!v 630 persons. Because of the <br />increase in[ people served and the development of several new <br /> <br />programs in ~93, Rise transportation needs have also grown. <br /> <br />Currently Rise transports 6 persons from Ramsev daily, with its <br /> <br />vehicles. <br /> <br /> 4. How are they benefitting? Rise meets the vocational <br />rehabilitation, housinq, and supported livinc se_~vice needs of <br />individuals ~itb mental, physical and devetoomental disabilities <br />in Anoka, Me~Depin, and Ramsey Counties. Rise orovides job traininq <br />and emO!oyme~t Qoportunities which Drovide income, a sens~ of self'~ <br />worth -and connections with others in the community. Rise Days a <br /> <br />commensurateiwace based on industry standards, providinq a chance <br />fo~ people w~h"disabilities to better their standard of livinG, <br /> <br />thereby, increasing their independence and self-sufficiency Rise <br /> transportation to ove~ 200 individuals who cannot access <br /> <br />provides <br /> <br />other modes ~of transportation. Rise vehicles transport people <br />within a 40 ~ile radius to over 26 job sites in the community, <br /> <br />47 <br /> <br /> <br />
