City of Ramsey is location. <br />A2. See attached Description of Services for the Senior Outreach Program. <br />A3. Seniors and disabled adults in Ramsey (625 seniors as of 1990). <br />A4. They benefit by the assistance that the Outreach Worker gives them. She <br /> helps them gain access to services and programs which will enable them <br /> to remain in their own homes or in the homes of their families/adult <br /> children. This will prevent unnecessary or premature nursing home or <br /> hospital admission, which benefits the entire community. <br />A3. Approximately 95% of clients served make less than $9000 per year. <br /> <br />This is a one year project. <br />Project runs from September, 1994 to May, 1995. <br /> <br />The Senior Outreach Program addresses the p~ob.!ems that vulnerable or isolated <br />seniors and disabled adults face as they attempt to remain independent and <br />productive in the community. Many clients cope with a disability or a chronic <br />impairment which reduces typical access to services. Oftentimes, clients simply <br />need a helping hand with complex and overwhelming paperwork, transportation <br />or other needs. Sometimes the need is of a crisis'inature,uwhere the O-~reach <br />Worker needs to assist the client in obtaining food, shelter, or fuel. Other <br />needs are more preventative, which hopefully alleviate future crises. The <br />Outreach Worker also is of aid to families who have an older adult living with <br />them, and feel the stress that such a situation involves. <br /> <br />$3000 <br />$3000 or partial funding. <br />Other revenue from area cities' CDBG funding.
