PROPOSED CONDITIONAL USE PERMIT <br />ANOKA COUNTY CENTRAL COMMUNICATIONS <br />EMERGENCY RADIO RECEIVER TOWER <br /> <br />On _ 1990, a conditional use permit was issued to Anoka County Central <br />Communications to consn-uct and maintain an emergency radio receiver tower facility in the City of <br />Ramsey. The subject property is legally described as follows: <br /> <br />This permit is issued pursuant to Section 170.013D of the Ramsey City Code. This conditional <br />use permit is perpetual in it's duration so long as the conditions imposed herein are complied with. <br />The conditions of this permit are as follows: <br /> <br />1) <br /> <br />That the permittee shall keep the tower and appurtenant structures in good repair at <br />all times. <br /> <br />2) That the maximum height of the tower/antenna shall be 60 feet. <br /> <br />3) <br /> <br />That the subject property shall be maintained in a manner compatible with that of the <br />surrounding neighborhood. <br /> <br />4) <br /> <br />The conditional use permit granted herein is subject to review at any reasonable time <br />and shall be reviewed annually by City Staff and is subject to revocation for <br />violation pursuant to those procedures as required by City Code. <br /> <br />Anoka County Central Communications hereby ac-knowledges receipt of this conditional use permit <br />and that they have reviewed the conditions of this permit and have agreed that they will cause <br />compliance with the terms of this permit. <br /> <br />By: <br /> <br />As: <br /> <br />Of: <br /> <br />STATE OF MINNESOTA ) <br />COUNTY OF ANOKA ) <br /> <br />On this day of ,198 , before me a Notary Public, personally <br />appeared , the <br />of. , to me known to be the person described in and who <br />executed the foregoing instrument and acknowledged that he/she executed the same as his/her free <br />act and deed. <br /> <br />Notary Public <br /> <br /> <br />
