7.19 resided, to the extent that the information relates to the <br />7.20 applicant's eligibility to possess a firearm; and <br />7.21 (5) (6) a statement by the applicant that, to the best of <br />7.22 the applicant's knowledge and belief, the applicant is not <br />7.23 prohibited by law from possessing a firearm. <br />7.24 (b) The statement under paragraph (a), clause (4) (5), must <br />7.25 comply with any applicable requirements of Code of Federal <br />7.26 Regulations, title 42, sections 2.31 to 2.35, with respect to <br />7.27 consent to disclosure of alcohol or drug abuse patient records. <br />7.28 (c) An applicant must submit to the sheriff an application <br />7.29 packet consisting only of the following items: <br />7.30 (1) a completed application form, signed and dated by the <br />7.31 applicant; <br />7.32 (2) an accurate photocopy of a the certificate, affidavit, <br />7.33 or other document described in subdivision 2a, paragraph (c), <br />7.34 that is submitted as the applicant's evidence of training in the <br />7.35 safe use of a pistol; and <br />7.36 (3) an accurate photocopy of the applicant's current <br />8.1 driver's license, state identification card, or the photo page <br />8.2 of the applicant's passport. <br />8.3 (d) In addition to the other application materials, a <br />8.4 person who is otherwise ineligible for a permit due to a <br />8.5 criminal conviction but who has obtained a pardon or <br />expungement <br />8.6 setting aside the conviction, sealing the conviction, or <br />8.7 otherwise restoring applicable rights, must submit a copy of the <br />8.8 relevant order. <br />8.9 (e) Applications must be submitted in person. <br />8.10 (f) The sheriff may charge a new application processing fee <br />8.11 in an amount not to exceed the actual and reasonable direct cost <br />8.12 of processing the application or $100, whichever is less. Of <br />8.13 this amount, $10 must be submitted to the commissioner and <br />8.14 deposited into the general fund. <br />8.15 (g) This subdivision prescribes the complete and exclusive <br />8.16 set of items an applicant is required to submit in order to <br />8.17 apply for a new or renewal permit to carry. The applicant must <br />8.18 not be asked or required to submit, voluntarily or <br />8.19 involuntarily, any information, fees, or documentation beyond <br />8.20 that specifically required by this subdivision.
