I <br />I <br /> <br /> I <br />'i <br /> ! <br />I <br /> <br />I <br />I <br />I <br /> <br />I <br />I <br />i <br />I <br />I <br />I <br />I <br />I <br />I <br /> <br />investigation of the facts set forth in the application. <br />The Chief of Police and such other department heads shall <br />make a written recommendation to the City Council as to <br />the issuance or non-issuance of the license within thirty <br />days. <br /> <br />The completed application form shall contain all <br />information indicated, including: <br /> <br />(a) Name, place, date of birth, and street residence of <br /> the applicant. <br /> <br />(b) The business address and the name and address of the <br /> owner of the premises, and the exact legal <br /> description of the licensed premises. <br /> <br />(c) A statement as to whether, within the preceding five <br /> years, the applicant has been convicted of any law <br /> relating to theft, damage or trespass to property, <br /> sale of a controlled substance, or the operation of <br /> business; the nature of any such offense and the <br /> penalty assessed. <br /> <br />(d) Whether the applicant is a natural person, <br /> <br /> corporation, or partnership. <br /> <br /> (a) If the applicant is a corporation, the state <br /> <br /> of the incorporation and the names and <br /> addresses of all officers and directors. The <br /> Application shall be accompanied by the <br /> <br /> Articles of Incorporation and Secretary of <br /> State's Certificate of good standing. <br /> <br /> (2) If the applicant is a partnership, the names <br /> <br /> <br />
