Calendar of Events Regarding Mr. Guimont's Request for Disability Benefits 10/21/90 Date of injury 12/1 1/90 City Council Meeting Council awarded disability in the amount of $800.00/month for the months of November and December, 1990. Mr. Guimont to submit a copy of his 1989 Federal Income Tax form and Schedule C. 2/5/91 Personnel Committee Meeting Mr. Guimont attended and asked for consideration of his request for additional benefits. As Mr. Guimont had not submitted copies of tax return information, the Personnel Committee directed that Mr. Guimont present his tax returns to the Finance Officer for review and thereafter his case be presented to the Fire Board for their recommendation to council. 5/15/91 Fire Board Meeting The Finance Officer stated the requested information had just been received, and requested this case be rescheduled to allow for review and verification time. 8/28/91 Fire Board Meeting Finance Officer made recommendation as to disability benefits. Mr. Guimont did not attend the meeting and the Fire Board asked that another meeting be established and his presence be requested. 9/4/91 Letter to Mr. Guimont requesting certain information and advising of special meeting called for October 9, 1991 to review his request for benefits. 10/8/91 Finance Officer received a call from Mr. Guimont's attorney (Kay Otto) and they reviewed the information request pending. 10/9/91 Finance Officer received a call from Mr. Guimont's attorney requesting the case be put on the next Fire Board agenda (November 20, 1991) as this would permit sufficient time for her to gather the requested information. 10/9/91 Fire Board Special Meeting canceled in the late afternoon.
