Massage Therapists/Business Survey <br />8. The proposed ordinance contains language relating to when a massage establishment <br />license is not required. Please rank whether or not you feel the following exceptions are <br />appropriate. <br />(1) A health care <br />facility licensed and/or <br />owned by the State of <br />Minnesota <br />(2) A medical clinic or <br />hospital, in which a <br />massage is performed <br />by a state -licensed <br />physician, <br />chiropractor, <br />osteopath, podiatrist, <br />nurse, physical <br />therapist, or an <br />assistant working <br />under the direction of <br />such professionals <br />(3) A physical therapy <br />clinic or athletic <br />facility, so long as the <br />massage is performed <br />by a state -licensed <br />physical therapist, <br />athletic director, <br />coach or trainer <br />(4) A cosmetologist, <br />esthetician, <br />beautician, or barber, <br />and treatment is <br />limited to the scalp, <br />face, neck, shoulders, <br />associated with a <br />scalp massage or the <br />hands and elbows, <br />associated with a <br />manicure, or feet to <br />knees, associated <br />with a pedicure <br />Very Appropriate <br />0 <br />Approriate Somewhat Appropriate Not at all Approriate <br />0 <br />0 0 <br />0 0 0 0 <br />0 0 0 0 <br />Page 3 <br />
