CITY OF RAMSEY <br />CONDITIONAL USE PERMIT AGREEMENT <br /> <br />This contract~ entered into this ~ day of March, 1984, between the City of Ramsey, <br />hereinafter'l~efe,~red to as "City", and Ms. Darlene Dickman, hereinafter referred <br />to as "AppliCant!', in conjunction with the issuance to the applicant a conditional <br />use permit:to o~erate a counseling service at her premises located at 8451 - 160th <br />Circle NW, Ramsay, Minnesota 55303. <br /> <br />The conditi,olial use permit is hereby granted contingent upon the applicant's complying <br />with the following: <br /> <br /> 1) That tl{e applicant shall see patients by appointment only. <br /> <br /> Z) That the applicant shall provide off-street parking. <br /> <br /> 3) Th.at .t~e applicant shall not see patients before 9:30 a.m. in the morning <br />or after 4:O0'rp.mL in the afternoon. <br /> <br />5) <br /> <br /> 6) That t~e City reserves the right to inspect the premises at any <br /> reasonable <br />time should ,it ~eceive' any complaints or suspect any violations to this conditional <br />use permit 'agreement. <br /> <br />SUMMARY <br /> <br />The applicaIii U~derstands and agrees that the conditional use permit granted herein <br />is subject to review at any time and shall be reviewed annually by City Staff. <br /> <br />Any violation o{ the conditions described herein or any violation of any other City <br />ordinances nlay be cause for immediate revocation of this conditional use permit. <br /> <br />APPLICANT: <br /> <br />Date: : By: <br /> <br />Ms. Darlene Dickman <br /> <br />crrY OF R ssy; <br /> <br />Date: By: <br /> <br />Mayor <br /> <br />By: <br /> <br />City Administrator/Clerk <br /> <br /> <br />
