Each application and appeal shall be in writing and shall <br />contain such information as hereinafter prescribed. <br /> <br />Application for Permission to Use Alternate Materials and <br />Methods of Construction. An application for permission to usc <br />alternate materials or methods of construction shall first be submitted <br /> <br />i ! to the Building Official for approval or denial and shall include: <br /> <br /> 1. The name and address of the applicant, and if the applicant is <br /> acting as agent for another, the name and address of the person, <br /> finn, or corporation for whom the applicant is agent. <br /> <br /> 2. A description of the type of building or smacture and the work to <br /> be performed for which permission to use alternate materials or <br /> methods is requested. <br /> <br /> 3. A description of the use and occupancy of the building or <br /> structure. <br /> <br />A citation to the conu'olling section or sections of the Uniform <br />Building Code which specify the materials or methods for which <br />alternates are requested. <br />A description of the alternate materials or methods of <br />construction proposed to be used. <br /> <br />If a test is required by the Building Off'acial pursuant to Sections <br />106 and 107 of the Building Code, a report from an approved <br />agency which has tested the proposed alternate materials or <br />methods of construction. Such report shall include the findings, <br />conclusions, and recommendations, if any, of the approved <br />agency. <br /> <br />The decision of the Building Official shall be in writing and shall <br />be served upon the applicant by mail. <br /> <br />An applicant who is dissatisfied with a decision of the Building <br />Official may thereupon appeal to the Board as hereinafter <br />provided. <br /> <br /> for Interpretation of the Building Code. Any <br />person who desires an interpretation of the Building Code shall £u'st <br />submit a written request for an interpretation to the Building Official. <br />Such request shall include: <br /> <br /> The name and address of the applicant, and if the applicant is <br /> acting as agent for another, the name and address of the person, <br /> firm or corporation for whom the applicant is agent. <br /> <br /> A description of the type of building or structure and the specific <br /> phase of the work to be performed which gives rise to the <br /> request for an interpretation. <br /> <br />3.
