Section 26 -851. Massage Establishment Business License Application. <br />(a) Each business or individual intending to offer massage services to the public <br />or for monetary compensation or other consideration must first apply for a license <br />to perform such activities. <br />(b) Each application for a massage establishment business license must be made <br />to the City Clerk on such forms as the Clerk may make available. <br />(c) Each application must indicate whether the applicant is a natural person, a <br />corporation, a partner, or other form of organization. <br />(d) Each application must indicate the name of the business, if it is to be <br />conducted under a designated name or style other than the full individual name of <br />the applicant. <br />(e) If the applicant is a natural person, the application must indicate or include: <br />(1) The true legal name, date of birth, street address, and phone number of <br />the applicant. <br />(2) Whether the applicant is now, or has ever been, known by a name other <br />than the applicant's current true legal name, and, if so, what such name or <br />names were, what dates such name was used, and in what capacity or place <br />such name was used. <br />(3) The street addresses at which the applicant has lived during the last five <br />years. <br />(4) The type, name, and location of every business or occupation the <br />applicant has been engaged in during the preceding five years. <br />(5) Whether the applicant has ever been convicted of a crime other than a <br />traffic violation and, if so, the time, place, and offense for each. <br />(6) Whether the applicant has any training or experience in performing <br />massage services, including any certifications, degrees, diplomas, or <br />educational coursework. <br />(7) A copy of identification, such as a driver's license, for the applicant. <br />(f) If the applicant is a partnership, the application must indicate or include the <br />name of the managing partner(s) and the interest of each partner in the business. <br />(g). If the applicant is a corporation or other organization, the application must <br />indicate or include: <br />(1) The name of the corporation or other organization, and, if incorporated, <br />the state of incorporation. <br />(2) The name of the manager or proprietor or other agent in charge of the <br />business to be licensed and all information concerning said person(s) as is <br />required for individual applicants in subpart (e) of this section. <br />(3) If the applicant is a nonprofit corporation, the application shall include <br />a list of all members of such nonprofit corporation. <br />(h) Each application must indicate whether the applicant is licensed or has applied <br />to be licensed in other communities to operate similar businesses, and, if so, in <br />5 <br />
