(j) Each application must indicate a description and location of the premises to be <br />licensed. If the premises are not fully constructed and furnished at the time of the <br />filing of the application, a copy of the plans for construction and/or furnishing the <br />premises must be attached to the application. <br />(k) Each application must indicate a description of the services to be provided, <br />including specific techniques and equipment. <br /> (l) Each application must be accompanied by: <br />(2) If the applicant is operating under an assumed name, a copy of the <br />certification required by Minnesota Statutes Chapter 333 certified by the <br />Secretary of State. <br />(5) Written authorization for the city, its agents, and employees to seek <br />information and conduct an investigation into the truth of the statements set <br />forth in the application, the qualifications of the applicant for the license, <br />and the fitness of the premises of business as a massage establishment. <br /> <br />Massage Therapist License. <br />Section 26-852. <br /> <br />(a) Each person intending to offer massage services to the public or for monetary <br />compensation or other consideration must first apply for a license to perform such <br />activities. <br />(b) Each application for a massage therapist license must be made to the City <br />Clerk on such forms as the Clerk may make available. <br /> (c) Each application must include: <br />(1) All information and documentation required for a business license for a <br />natural person as required in subpart (e) of Section 26-851. <br />(2) The length of experience of the applicant in this occupation and the <br />past places of employment and positions held. <br />(3) The name and address of any training institutions attended and the <br />dates of such attendance. <br />(d) Each application must indicate whether the applicant is licensed or has applied <br />to be licensed in other communities to perform massage services, and, if so, in <br />which community the applicant is licensed or has applied and the status of such <br />license or application. <br />(e) Each application must indicate whether the applicant has previously been <br />denied a license to perform massage services, or has had such a license revoked or <br />suspended, and, if so, the circumstances of such denial, revocation, or suspension. <br />(f) Each application must indicate a description of the services to be provided, <br />including specific techniques and equipment. <br />(g) Each application must indicate a description and location of the premises at <br />which the applicant will perform massage services. <br />(h) Each application must be accompanied by written authorization for the City, <br />its agents, and employees to seek information and conduct an investigation into <br />6 <br /> <br /> <br />
