',..

Richard returns today with silnilar neck and low back symptoms as
to those he has continued to have since I first saw him back in
October of 1981. tte is having a lot of trouble with his right
sl~oulder now; this bothers him at night. It limits his movement
such as reaching into his back pocket or abducting his arm from
his side. He is taking Anacin and Motrin at the same time.
I have warned him about the dangers of this. He still gets a
lot of relief from his TENS unit. He has some prostatism symptoms
for which he is going to attend his own doctor. His left knee
still bothers him. His wife does most of the work around the
l~ouse. I have given him a note today stating that he has been
totally disabled since October of 1981 when I last saw him, so
that he can collect on insurance for loan payments.

Itis examination today remains stable. I would like to see the
patient again in three months. I see no signs of progression
of his condition. I still feel that he is totally unable to
return to any type of regular work.

SFN/mkm

cc City of Ramsey, Street Department, 15133 Nowthen Boulevard, Anoka,
Mn. 55303
