Period 12 began on October 1, 1980 and ended on September 30, 1981. Regular revenue sharing payments are made quarterly on the fifth working day following the end of the quarter to which they apply. Statement of Assurances for Period 13 Mailed I I I I I By now your government should have received its Statement of Assurances form for Period 13 (October 1, 1981 - September 30, 1982). The forms were mailed first-class from the mid-west in mid-September. The suggested due date for return mail of the form was September 30, 1981 to allow sufficient time for processing before the first payment in early January, 1982. Governments for which forms have been received will be sent a receipt card within a month after*the form arrives here. If your government has not yet signed and returned the form~ please do so now. If your government has not received its assurances form, which was sent to the official address of record for your govern- ment, please contact our Intergovernmental Relations Division at the letterhead address or by telephone at (202) 634-5200 [not a toll-free number] to arrange for a replacement assurances form to be sent to you. Census Revenue Sharin8 Forms Governments which report revenue sharing and other data directly to the Bureau of the Census were sent the proper form in mid-September on which to report this information. A copy of the report must be made availabl:e for public inspection, together with supporting documentation, within 10 days after returning the form to the Census Bureau. A newspaper notice advising the public of the availability of the report form for inspection must also be published promptly after the forms are returned. In a majority of States, most local governments will receive survey forms from a State or county government agency which collects these data by arrangement with the Census Bureau. These governments· may receive their forms at a.later date. Most governments will be asked to complete the same forms they received las~ year. The Census Bureau and other data collection agencies working in cooperation with it may send these form~ to someone other than the chief executive of your government° Please determine who in your government has completed these forms in the past and encourage that person to prepare.the required report as soon as accurate figures are available. It is not necessary that the report be delayed until an audit is conducted so long as accurate information is reported.
