OFFIC£ OF REVENUE :'401 E STREET, <br />JCOLUHBIA PLAZA HIGHRISE <br />WASHINGTON, D.C. 20226 <br /> <br />OFFICE OF THE SECRETARY OF THE TREASURY <br /> <br />October 7, 1981 <br />REF.' 5132-P <br /> <br />Dear Chief Executive: <br /> <br /> Today we made the fourth and final quarterly revenue sharing <br />payment to your government for Entitlement Period 12. Period 12 began <br />on October 1, 1980 and ended on September 30, 1981. Regular revenue <br />sharing payments are made quarterly on the fifth working day following <br />the end of the quarter to which they apply. <br /> <br />Statement of Assurances for Period 13 Mailed <br /> <br />I <br />I <br />I <br />I <br />I <br /> <br /> By now your government should have received its Statement of <br />Assurances form for Period 13 (October 1, 1981 - September 30, 1982). <br />The forms were mailed first-class from the mid-west in mid-September. <br />The suggested due date for return mail of the form was September 30, <br />1981 to allow sufficient time for processing before the first payment in <br />early January, 1982. Governments for which forms have been received <br />will be sent a receipt card within a month after*the form arrives here. <br /> <br /> If your government has not yet signed and returned the form~ <br />please do so now. If your government has not received its assurances <br />form, which was sent to the official address of record for your govern- <br />ment, please contact our Intergovernmental Relations Division at the <br />letterhead address or by telephone at (202) 634-5200 [not a toll-free <br />number] to arrange for a replacement assurances form to be sent to <br />you. <br /> <br /> Census Revenue Sharin8 Forms <br /> <br /> Governments which report revenue sharing and other data directly <br />to the Bureau of the Census were sent the proper form in mid-September <br />on which to report this information. A copy of the report must be made <br />availabl:e for public inspection, together with supporting documentation, <br />within 10 days after returning the form to the Census Bureau. A newspaper <br />notice advising the public of the availability of the report form for <br />inspection must also be published promptly after the forms are returned. <br /> <br /> In a majority of States, most local governments will receive <br />survey forms from a State or county government agency which collects <br />these data by arrangement with the Census Bureau. These governments· <br />may receive their forms at a.later date. Most governments will be <br />asked to complete the same forms they received las~ year. <br /> <br /> The Census Bureau and other data collection agencies working <br />in cooperation with it may send these form~ to someone other than <br />the chief executive of your government° Please determine who in your <br />government has completed these forms in the past and encourage that <br />person to prepare.the required report as soon as accurate figures <br />are available. It is not necessary that the report be delayed until <br />an audit is conducted so long as accurate information is reported. <br /> <br /> <br />
