REQUEST FOR QUOTES May 9,2016 <br /> FOR <br /> ASBESTOS ABATEMENT AND <br /> REGULATED WASTE REMOVAL <br /> CITY OF RAMSEY <br /> ANOKA COUNTY,MINNESOTA <br /> I. INTRODUCTION <br /> The City of Ramsey, Anoka County, Minnesota is soliciting Quotes for a contract to have <br /> an independent third party provide asbestos abatement and hazardous waste removal <br /> services at the former City Hall building located at 15153 Nowthen Blvd NW and the <br /> former Police Department building located at 15240 Helium Street NW, Ramsey MN <br /> 55303 (see attached exhibit). <br /> One copy of the Quote must be submitted. The Quote must be submitted by 4:30 pm on <br /> May 27, 2016. All Quotes must be signed and submitted on the attached Quote Form. <br /> Quotes may be mailed, delivered, faxed, or emailed to Shane Nelson, consultant engineer. <br /> All Quotes are to be made to: City of Ramsey, 7550 Sunwood Drive, Ramsey MN 55303 <br /> II. GENERAL REQUIREMENTS <br /> 1. The Contractor shall furnish all labor, tools, materials, equipment and supervision <br /> necessary for the performance of all operations. <br /> 2. Upon acceptance of a quote, the Contractor shall enter into a contract with the City for <br /> the performance of the work. <br /> 3. The Contractor shall be a licensed Asbestos Abatement Contractor with the Minnesota <br /> Department of Health and shall provide a copy of its license to the City. <br /> 4. The Contractor must apply for and receive a City of Ramsey Contractor's License (see <br /> attached application) before beginning the work. Contractor is responsible for payment <br /> of the $50 license fee. <br /> 5. The Contractor shall provide proof of insurance. The limits of liability for the insurance <br /> required shall provide coverage for not less than the following amounts or greater <br /> where required by Laws and Regulations: <br /> a. Workers' Compensation, and related coverages: <br /> 1) State: Statutory <br /> 2) Applicable Federal <br /> (e.g.,Longshoreman's): Statutory <br /> 3) Employer's Liability: $500,000 <br /> 1 <br />
