During the administrative conference the following agreement w~ <br />reached by the parties: <br /> <br />The insurer agrees to pay $7,226.88 as a full, final <br />and complete settlement for prescribed medical <br />equipment in this case. Of the above-noted amount, <br />$6,226.88 is a 75 percent compromise of the current <br />outstanding medical bills submitted by the employee in <br />the amount of $8,302.50. The additional $1,000.00 <br />will be paid to Medi-Rents for purchase of the <br />employee's current used hospital bed and ancillary <br />medical equipment. <br /> <br />This agreement involves only the following medical <br />equipment: <br /> <br />1. A fully electrical hospital bed with <br />hand held controls for head, knee and bed <br />height and manual emergency power out <br />bypass; orthopedic innerspring mattress <br />(gatch type) with staph check. <br /> <br />2. Full overhead traction/fracture frame <br />with orthopedic pull and chain. <br /> <br />3. Side rails for hospital bed. <br /> <br />4. Bucks triple pulley pelvic traction. <br /> <br />5. Orthopedic laminectomy chair. <br /> <br />ORDER: <br /> <br />1. The agreement reached during the administrative conference is <br />legally binding. Thereby, all parties are expected to abide by it. <br /> <br />2. Any appeals from this Decision and Order must be filed with the <br />Medical Services Review Board within thirty (30) days from the da~e of <br />service of this order.
