I <br />I <br />I <br />I <br />I <br /> <br /> CITY OF RAMSEY <br />CONDITIONAL USE PERMIT AGREEMENT <br /> <br />This contract, entered into this __ day of March, 1984, between the City of Ramsey, <br />hereinafter referred to as "City", and Ms. Darlene Dickman, hereinafter referred <br />to as "Applicant", in conjunction with the issuance to the applicant a conditional <br />use permit to operate a counseling service at her premises located at 8451 - 166th <br />Circle NW, Ramsey, Minnesota 55303. <br /> <br />The conditional use permit is hereby granted contingent upon the applicant's complying <br />with the following: <br /> <br />1) That the applicant shall see patients by appointment only. <br /> <br />Z) That the applicant shall provide off-street parking. <br /> <br /> 3) That the applicant shall not see patients before 9:30 a.m. in the morning <br />or after 4:00 p.m. in the afternoon. <br /> <br />4) <br /> <br />5) <br /> <br /> 6) That the City reserves the right to inspect the premises at any reasonable <br />time should it receive any complaints or suspect any violations to this conditional <br />use permit agreement. <br /> <br />SUMMARY <br /> <br />The applicant understands and agrees that the conditional use permit granted herein <br />is subject to review at any time and shall be reviewed annually by City Staff. <br /> <br />Any violation of the conditions described herein or any violation of any other City <br />ordinances may be cause for immediate revocation of this conditional use permit. <br /> <br />APPLICANT: <br /> <br />Date: By: <br /> <br />Ms. Darlene Dickman <br /> <br />CITY OF RAMSEY: <br /> <br />Date: <br /> <br />By: <br /> Mayor <br /> <br />By: <br /> <br />City Administrator/Clerk <br /> <br /> <br />
