Upon Completion of site activities EMS will provide a final report with applicable <br />transportation and disposal documentation. Terms of payment: Net 30 days of invoice. <br />Scope of work notations: <br />a) EMS reserves the right to request a written change order in the event additional unidentified or unusual conditions are <br />discovered during the demolition process. <br />b) Hazardous and or regulated materials are not addressed within this proposal. However, upon written request, a cost may <br />be provided to you and the City of Ramsey. <br />c) Proposal based upon having all utilities disconnected prior to EMS performing the work outlined above, including but not <br />limited to Electrical, Gas, and Water services. <br />d) The City of Ramsey is to supply EMS with a copy of their Pre -Demolition Hazardous Materials Assessment for the purpose <br />of applying for the ten (10) day MPCA Notification of Intent to Perform a Demolition application. <br />AUTHORIZED BY: TITLE: SIGNATURE: <br />Eric L.
