Inc. <br />222 Monroe Street, Anoka, Minnesota 55303 <br /> <br /> ! <br /> I <br /> I <br /> i <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />i <br />I <br /> <br />Date Received June 12, 1986 <br /> <br />Plan File No. A-7430 <br /> <br />Ownership - Municipal <br /> <br />Scope - This report covers the design of this project insofar as the safety and <br />sanitary quality of the water for public consumption may be affected, and applies <br />to this project only and not to the entire system or any other part thereof. <br />This project consists of the installation of a municipal veil with the following <br />apecifications~ <br /> l) 4~0 feet deep <br /> 2) 20 inch x 14 inch steel cased <br /> 3) 500 <br /> <br />Compliance - NO CONSTRUCTION SHALL TAKE PLACE EXCEPT IN ACCORDANCE WITH THE APPROVED <br />PLANS. If it is desired to make deviations from the approved plans, the State <br />Department of Health should be consulted and approval of the changes obtained before <br />construction is started; otherwise, such construction is installed without proper <br />approval, and in addition may create dangers to public health. <br /> <br />Conclusion <br /> <br />The plans and specifications on this waterworks project are in general accordance <br />with the requirements of the Minnesota Department of Health, and are recommended <br />for approval with the understanding as stated in the preceding paragraphs, and with <br />the usual reservations as stated on the appended sheet, entitled "Information <br />Relative to Plan Examination." <br /> <br />(See other side) <br /> <br />Approved: <br /> <br /> ~..-- ,,_ .:~.~.- .~ ....~'-":.~..-.< .... <br />Richard D.
