The applicant or Its contractor shall notify Hr. Forest Noen or Nr. Gordon 14cKInnon, et 545-~76i, et least one weak prior to starting construction so State Inspection may be arranged~ end for approval of ell hlgh~ay materials prior to their piece, ant. The total Installation shall ~eet HR/DOT speclflcetlons. The applicant shell provlde In-house Inspection at ell times while on trunk hlgh~ay right of ~lnter work, November I~ through April 15 shall be considered winter ~ork~ · ork during this perlod shell be on a day-to-day basls et the discretion of the Dlstrlct Halntenence Engineer. The location of the access Is approved es sho~n on the applicant's attached plea. The design of the access Is approved as shown on the State's drlve~ey construction fo~m. The access shall be bituminous surfaced within the State's right-of-way. All bituminous surfacing shell extend to and match the laplace blt~Inous roadway. The outside edge of the laplace bituminous roadway shall be sew-cut to provlbe a ual form edge for the ne~ construction to abut. The State shall furnish 50* X ze' C.H. culvert. The applicant shall notify Nr. Tom Anderle, et 545-~761~ at least one week before the culvert 'Is needed to arrange for del Ivery. All disturbed turf areas shell be covered with a mlnlmum of 6 Inches of slope dressing and shall either be seeded with HhL/DOT Spec. :~876 Type 5 seed mixture, or be sodded. The seed mixture shall be applied et the rate of 75 pounds per acre, Fertilize ell areas wlth commercial fertilizer analysis 10-20-20, et a rate of 450 pounds per acre, or equivalent. All sod shell be pegged. All seeded areas shall be covered with disc anchored mulch materiel. The mulch materiel shall consist of grain stra~, hey~ cuttings of agricultural grasses, end legumes. The materiel shall be relatively free of seed bearing stalks of noxious grasses or weeds as defined by the rules end reguatlons of the Hlnnesofa Oepar~nent of Agriculture. The rate of application shell be approximately 2 tons per acre, The applicant shall furnish end Install ell traffic signing as specified on the attached D.T.~.O. Signing Layout, Standard Sign Dra~Ings, and Sign Installation Specifications p~phlet. The applicant shall contact the I~t/DOT Traffic Operations Section0 at §4~-~5761 ext. 121, et least 48 hours prior to performing this work to arrange for Inspection.
