2) City contributions for 2018 health insurance, as follows: <br /> The employer will make the following contributions toward group health insurance <br /> coverage for employees enrolled in the City's plan during 2018. In addition, the City will <br /> purchase $20,000 of basic life insurance for full-time regular employees. <br /> The City contribution to the HRANEBA or H.S.A. is included in the figures listed. The <br /> $4000 deductible single plan receives $194.00 per month toward the HRANEBA or <br /> H.S.A. The other single plans receive $130.00 per month toward the HRANEBA or <br /> H.S.A. All employee +children and employee + spouse plans will receive $160.00 per <br /> month toward the HRANEBA or H.S.A. Family plans will receive $192.00 per month <br /> toward the HRANEBA or H.S.A. <br /> City Contributions <br /> Employee only coverage is from$700.00 to $723.50, plus up to $53.42 per month. <br /> Employee and Children coverage is $871.60, plus up to $64.04, Employee and Spouse <br /> coverage is $907.30,plus up to $67.26 and Family coverage is $1116.90, plus up to <br /> $83.24. <br /> Amount of Increase for 2018 (over 2017 amounts) -Not to Exceed <br /> Single Premium increased by up to $53.42 <br /> Employee + Spouse increased by up to $67.26 <br /> Employee + Children increased by up to $64.04 <br /> Family increased by up to $83.24 <br /> Waiver/Credit in lieu of Coverage --No Change <br /> Note: The rate of premium increase in 2018 is capped at 9%. In the very unlikely event <br /> of a premium reduction in 2018, in no event will any potential 2018 premium savings be <br /> returned to employees in the form of payment. <br /> Or <br /> Provide cash in lieu of City's insurance contribution of$370 per month in 2018. <br /> Employees receiving the waiving benefit prior to 2013 will continue to be grandfathered <br /> in to the waiving benefit.
