We have reviewed the May 23, 2019 plans for the project. <br />SCOPE OF WORK <br />Bolton & Menk will complete the following survey related tasks associated with the project: <br />• Establish Horizontal and Vertical Control, <br />• Create Stakeout Files, <br />• Storm Sewer — 300 ft, 3 structures, <br />• Pumping Station and Valve Vault, <br />• Access Road (800 ft — centerline), <br />• Ponds (1 contour around each pond), and <br />• As -built survey for the project when completed. <br />ASSUMPTIONS <br />We have made the following assumptions in preparing this proposal: <br />• We will be supplied with AutoCAD files of the design, including alignments and profiles of the <br />storm sewer, <br />• The As -Built Survey will consist of rim and invert elevations for the storm sewer, and contours <br />for the ponds. <br />• The As -Built information will be provided to the City in XYZ format. <br />PROPOSED FEES <br />Bolton & Menk proposes to perform the staking based on the hours and rates presented in the following <br />table: <br />Personnel Hours Rate/Hr Total <br />Survey Technician 6.0 $100.00 $ 600.00 <br />Survey Crew Chief 20.0 $115.00 $ 2,300.00 <br />Totals 26.0 $ 2,900.00 <br />C:AUsers\kevinki\DesktopA2019 Construction Staking Proposals\Ramsey - 2019 - 19-03\Ramsey 2019 COR Infiltration CP 18-09 Construction Staking.docx <br />Bolton & Menk is an equal opportunity employer. <br />
