DEPARTMENT <br />OF HEALTH <br />SWIFT Contract Number [186689] <br />Between the Minnesota Department of Health and [City of Ramsey] <br />Minnesota Department of Health <br />Grant Agreement <br />This grant agreement is between the State of Minnesota, acting through its Commissioner of the <br />Department of Health ("MDH") and City of Ramsey ("Grantee"). Grantee's address is 7550 Sunwood <br />Drive NW, Ramsey, Minnesota 55303. <br />Recitals <br />1. MDH is empowered to enter into this grant agreement under Minn. Stat. § 144.0742 and <br />§114D.50 Clean Water Fund . <br />2. MDH is in need of assisting public water suppliers to protect the source of drinking water. <br />3. The vision of the Department of Health is for health equity in Minnesota, where all <br />communities are thriving and all people have what they need to be healthy. Health equity is <br />achieved when every person has the opportunity to attain their health potential. Grantee <br />agrees, where applicable, to perform its work with advancing health equity as a goal. <br />4. Grantee represents that it is duly qualified and will perform all the duties described in this <br />agreement to the satisfaction of MDH. Grantee agrees to minimize administrative costs as a <br />condition of this grant pursuant to Minn. Stat. § 16B.98, subd 1. <br />Grant Agreement <br />1. Term of Agreement <br />1.1. Effective Date <br />January 8, 2021, or the date MDH obtains all required signatures under Minn. Stat. <br />§ 16B.98, subd. 5, whichever is later. Per Minn. Stat. § 16B.98, subd 7, no payments will <br />be made to the Grantee until this grant contract is fully executed. Grantee must not <br />begin work until this contract is fully executed and MDH's Authorized Representative <br />has notified Grantee that work may commence. <br />1.2. Expiration Date <br />December 31, 2021, or until all obligations have been fulfilled to the satisfaction of <br />MDH, whichever occurs first. <br />1.3. Survival of Terms <br />The following clauses survive the expiration or cancellation of this grant contract: <br />8. Liability; 9. State Audits; 10. Government Data Practices and Data Disclosure; 11. <br />Ownership of Equipment; 12. Intellectual Property; 14. Publicity and Endorsement; and <br />16.
