PROJECT LOCATION: The City of Ramsey (City) is requesting quotes from Contractors (Contractor) to patch potholes greater than 2-inches deep on the streets as identified and prioritized in Figure 1 (attached). PROJECT SCOPE: The overall scope of this work includes patching potholes greater than 2- inches deep on approximately 10 miles of City streets identified by orange highlighting and prioritized by area from 1 through 7 in Figure 1, with area 1 having the highest priority and area 7 having the lowest priority. Work must start in area 1 and progress to area 7. Debris and water shall first be removed from potholes using compressed air, then CSS-lh tack material shall be applied to all clean and dry bituminous surfaces within potholes. Potholes shall then be filled with V2-inch bituminous mixture. Compaction of the bituminous mixture shall be achieved using a one (1) ton roller. Plate compactors may be used if a 1-ton roller is unable to achieve compaction. Once work has commenced, the Contractor shall continuously pursue completion until work is complete or the total value of work completed reaches $150,000. DELIVERABLES: • Signature Page —The enclosed Signature Page must be completed in its entirety and submitted, including signature(s). • Quote Form -The enclosed Quote Form must be completed in its entirety and submitted. COMMUNICATION WITH CITY: If any requirements of this RFQ are not clearly understood, Contractor is responsible for promptly contacting City for further clarification. Responses to inquiries that substantially modify or clarify the RFQ will be forwarded by addenda. SUBMISSION OF QUOTE: A signed copy of the signature page, and attached quote form, must be submitted to the City by 10:00 AM, Friday, May 5, 2023. Deliver to the Ramsey Municipal Center, 7550 Sunwood Drive NW, Ramsey, MN 55303 to the attention of Bruce Westby, City Engineer/Public Works Director, bwestby cityoframsey.com. Quotes received by the submittal deadline via e-mail will be accepted. PROPOSAL UNDERSTANDING: By submitting a quote, Contractor agrees to be bound to the terms and conditions herein. COSTS OF PREPARATION: Under no conditions will any costs for preparation of the quote be reimbursed by the City. LICENSE AND INSURANCE: By submitting a quote, Contractor affirms they shall maintain the following minimum insurance coverage while completing their work as related to this RFQ in the following amounts: Commercial General Liability $1,000,000 per occurrence (written on an occurrence -based form bodily injury and property damage); automobile liability $1,000,000 per occurrence (including hired &non -owned bodily injury and property damage). The City of Ramsey shall be named as an additional insured on the Commercial General Liability insurance. Certificates of Insurance for all of the above insurance shall be filed with the City prior to the work.
