Name: <br /> <br />Storm Drainage Utility Credit Application Form <br /> <br /> Utility Account Number: <br /> <br />Billing Address: <br /> <br />Telephone Number: <br />Property Address: <br /> <br />Anoka County PIN: <br /> <br />This application is strictly voluntary. Each property has its own characteristics that distinguishes it from <br />other properties in the City. This application form is used in the event a property has on-site measures <br />that either decrease the amount of runoff generated or increase the water quality of the runoff. In order to <br />be considered for a storm drainage utility credit, the property owner will be required to supply ALL of <br />the information, including engineering calculations, to justify the credit. If a property owner or person <br />responsible for paying for the storm drainage fee believes that a particular fee is incorrect, such person <br />may request in writing that the fee be recomputed. If an error of more than 20% has been made, the fee <br />will be adjusted effective the quarter after receipt of the letter. No adjustments will be made <br />retroactively. <br /> <br />Requffed Submitra~: <br /> <br />Provide one accurately scaled and dimensioned site drawing showing size and location of all property <br />lines, streets, buildings, and structures; parking lots, driveways, and other improved surfaces; location, <br />size and grade of all drainage facilities. <br /> <br />Drawing shall show site contours and elevations of sufficient detail to accurately determine flow <br />directions and areas. <br /> <br />Calculate and/or provide the following information: <br /> <br />1. Total site area in square feet: <br /> <br />2. Total improved surface area: <br /> <br />3. Total pement improved: <br /> <br />A. Modified Method (at least 20% difference required) <br /> <br />For those properties that exceed 20 acres in size and have an overall percent impervious less than i0%, <br />the property owner may request that the City modify the approach taken to calculate the storm drainage <br />fee for their property. The modified approach to the storm drainage fee is calculated by assuming that a <br />single rectangular area which completely encompasses ALL of the on-site impervious area be regarded <br />as the modified area.
