CouncilmeJnbcr DcLuca introduceA the following resolution and moved for its adoption: RESOLUTION #90-09-307 RES()LIJTi~(~N APPROVING MEDICAL LEAVE OF ABSENCE FOR ROGERS GAYLE W!-II,;I,:E4S, the labor agreement between the City of Ramsey and the AFSCME bargaining inJiJ, Council No. 14, Local No. 2454, provides for leaves of absence when granted by the employer (t ~i~y of Ramsey); and WHER~;&S, Gayle Rogers, Accounting Technician in the Finance Department, has requested aie. avc of absence for medical leave; and the effective date for the medical leave is September 4, 1990; and WII!!V,I,{E&S, Ms. Rogers has presented the City of Ramsey with a note from her physician stath~g !hat the approximate time of recovery will be six weeks with an additional four weeks of padia~ m,;overy; and WH Ell E AS, the length of the medical leave of absence and the employee's return to work date will be dct,~mi ned by the employee's physician; and WIiI,;I~:E&S, Ms. Rogers will use (in this order) any earned but unused sick leave, com pensaio? ~i~ r,c and vacation and requests any remaining days as unpaid leave of absence; and WI[~;R I!;?~ S, all employees must carry, at minimum, single health insurance coverage and the emplo)/cc & ~e:~ nor contribute toward the employee insurance allowance during unpaid leaves of absence; one Wl~.
