CouncilmeJnbcr DcLuca introduceA the following resolution and moved for its adoption: <br /> RESOLUTION #90-09-307 <br /> <br />RES()LIJTi~(~N APPROVING MEDICAL LEAVE OF ABSENCE FOR <br />ROGERS <br /> <br />GAYLE <br /> <br /> W!-II,;I,:E4S, the labor agreement between the City of Ramsey and the AFSCME <br />bargaining inJiJ, Council No. 14, Local No. 2454, provides for leaves of absence when granted by <br />the employer (t ~i~y of Ramsey); and <br /> <br /> WHER~;&S, Gayle Rogers, Accounting Technician in the Finance Department, has <br />requested aie. avc of absence for medical leave; and <br /> <br />the effective date for the medical leave is September 4, 1990; and <br /> <br /> WII!!V,I,{E&S, Ms. Rogers has presented the City of Ramsey with a note from her <br />physician stath~g !hat the approximate time of recovery will be six weeks with an additional four <br />weeks of padia~ m,;overy; and <br /> <br /> WH Ell E AS, the length of the medical leave of absence and the employee's return to work <br />date will be dct,~mi ned by the employee's physician; and <br /> <br /> WIiI,;I~:E&S, Ms. Rogers will use (in this order) any earned but unused sick leave, <br />com pensaio? ~i~ r,c and vacation and requests any remaining days as unpaid leave of absence; and <br /> <br /> WI[~;R I!;?~ S, all employees must carry, at minimum, single health insurance coverage and <br />the emplo)/cc & ~e:~ nor contribute toward the employee insurance allowance during unpaid leaves of <br />absence; one <br /> <br /> Wl~.
