Councihne;~&,:r l'earson introduced the following resolution and moved for its adoption: <br /> <br />RESOLUTION #90-12-371 <br /> <br />RESOLUTI('O4 APPROVING LEAVE OF ABSENCE FOR MEDICAL LEAVE FOR <br />JO ANN l~lklJN(, <br /> <br /> W[~EREAS, consistent with City of Ramsey past practices, medical leaves of absence <br />have been ~r',mtt'~i by the empk)yer (City of Ramsey); and <br /> <br /> lA/t [ E II I,; A S, Jo Ann Thieling, Recording Secretary for Administrative Services, has <br />requested a l,:avc ,:)f absence for medical leave; and <br /> <br />the effective date for the medical leave is December 6, 1990; and <br /> <br /> WID,;RE AS, Ms. Thieling has presented the City of Ramsey with a note from her <br />physician sla~iW, the approximate time of recovery will be three to four weeks; and <br /> <br /> WI IEI;~EAS, the length of the medical leave of absence and the employee's return to work <br />date will be cicic~ mined by the employee's physician; and <br /> <br /> WII I,;t4EAS, Ms. Thieling will first use any earned but unused sick leave, compensatory <br />time and ,/aca~ion time; in that specific order and requests that any remaining days as unpaid leave <br />of absc~c{;; :tt~d <br /> <br /> WIIi,;RE?.S, all employees must carry, at minimum, single health insurance coverage and <br />the employer &w.s ~ot contribute toward the employee insurance allowance during unpaid leaves of <br />absence; a,M <br /> <br /> WIIEREAS, should it be necessary, the employee will be responsible for the actual <br />insurance c,. ,st ~{> ti~c Ciiy during any unpaid leaves of absence. <br /> <br />NOW TllERF.
