1 <br />(F) <br />Zoning Map Amendments <br />2 <br />(G) <br />Comprehensive Plan Amendments <br />3 <br />(H) <br />Home Occupation Permits (Level 11) <br />4 <br />(1) <br />Easement Vacations <br />5 <br />(J) <br />Encroachment Agreements <br />6 <br />7 <br />211 General information provided in an application form <br />8 <br />All requests for land use approvals required within this chapter shall be filed with the City on an official <br />9 <br />application form and must be submitted to the zoning administrator by deadlines set forth in an adopted <br />10 <br />calendar. Failure to submit a completed form is grounds for rejection of an application. The following <br />11 <br />information is required on all land use application forms: <br />12 <br />(A) <br />The applicant's name, business name, address, telephone number, and email address. <br />13 <br />(B) <br />The property owner's name, business name, address, telephone number, and email address. <br />14 <br />(C) <br />Signatures of the applicant and property owner consenting to the application. <br />15 <br />(D) <br />The address and/or legal description of the subject site. <br />16 <br />(E) <br />The land use application type(s) being sought. <br />17 <br />(F) <br />A written narrative explaining the request. <br />18 <br />(G) <br />The applicant's and/or owner's designated representative's business name, address, telephone <br />19 <br />number, and email address. <br />20 <br />(H' <br />Consent from the homeowner's association or architectural review board, if applicable. <br />21 <br />22 212 Information provided on plans <br />23 The information required for all land use application types generally consists of the following items, as <br />24 applicable to the application, and shall be submitted when applicable or requested by the zoning <br />25 administrator. Plans must be prepared in a standard engineering or architectural scale and be prepared <br />26 by a licensed surveyor, architect, and/or civil engineer, as applicable. All plans, with the exception of <br />27 architectural renderings, shall be in black, greyscale, and white. <br />28 (A) Existing Conditions. <br />29 (1) Affected parcel(s) legal description, dimensions, and area. <br />30 (2) Location of all existing significant trees, type, diameter, and which trees will be removed. <br />31 (3) Location of all existing structures on the site. <br />32 <br />33 . <br />34 (5) Existing elevation contours at two -foot intervals extending 100 feet outside of the property <br />35 boundary. <br />36 (6) Floodplain and wetland boundaries. <br />37 (B) Proposed Site Plan. <br />38 (1) Location of all existing and proposed structures. <br />39 (2) Location and dimensions of vehicular circulation, curb cuts, driveways, drive aisles, and <br />40 number of existing and proposed parking spaces. <br />41 (3) Calculations of parking space requirements. <br />42 (4) Site plan details such as trash enclosures, etc. <br />43 (5) Setbacks of all buildings, parking areas, and driveways from all property lines <br />44 (6) Snow storage locations. <br />45 (C) Proposed Architectural Plans. <br />46 (1) Color architectural elevations with type and materials used in all exterior surfaces. <br />47 (2) Location and type of all proposed wall -mounted lights. <br />48 (3) Dimensions of proposed and existing structures. <br />49 (4) "Typical" floor plan and "typical" room plan. <br />Page 28 of 160 <br />
