Suite 204,Ramsey MN 55303. <br /> Notification: <br /> Applicant has been notified of denial recommendation via email on October 4, 2023 along with a mailed letter. <br /> Recommendation: <br /> After completion of the required Background Check, Chief Katers recommends denial of the licenses for <br /> Therapeutic Massage Establishment for Creekside Relaxing Spa LLC located at 6957 Highway 10 NW. Education requirements of city ordinance are not satisfied as the school listed is not accredited by the U.S. <br /> Department of Education. <br /> 2. Residence history listed in the application does not coincide with locations worked or schooling provided in <br /> employment history and schooling records. <br /> 3.
