Indemnification Agreement. The permittee shall provide a signed <br />agreement to the department which indemnifies and saves the city, holding <br />it harmless from all losses, damages, costs and charges that may be <br />incurred by the city due to the failure of the permittee to conform to and <br />comply with the provisions of this ordinance. <br /> <br />d. Permit Application Requirements. <br /> <br />1. All applicants for an ISTS permit shall include the following information: <br /> <br />(a) <br />(b) <br />(c) <br />(d) <br /> <br />(e) <br />(0 <br /> <br />(g) <br />(h) <br />(i) <br /> <br />Name and address of property owner; <br />Property identification number; <br />Legal description of the property; <br />ISTS Designer Name, address, phone number and State ISTS <br />License number; (or department qualified employee name and <br />number); <br />Site evaluation report on a form as provided by the city; <br />System design with full information including applicable <br />construction information on forms as provided by the city, and <br />Any other information requested pertinent to the process; <br />A certified statement from the person who conducted the work; <br />On lots created after January 23, 1996, the system design shall <br />include at least one designated additional soil treatment area which <br />can support a standards soil treatment system. <br /> <br />Application Review and Approval. If, after consideration of the <br />application for a permit, a qualified employee or authorized licensee of the <br />department shall be satisfied that the work contemplated conforms to and <br />complies with provisions of this SECTION, the department shall issue a <br />written permit granting preliminary approval authorizing initiation of <br />construction of the system as designed. <br /> <br />Incomplete Application Information. If after consideration of the <br />application for a permit, the department shall be satisfied that the work <br />contemplated with not conform to or comply with the provisions of this <br />SECTION, the department shall deny the application for a permit. Notice <br />of such denial shall be served on the applicant or permittee. The notice <br />shall state the reason for denial. The permit application may be revised or <br />corrected and resubmitted to the department at any reasonable time for <br />reconsideration. <br /> <br />e. Fees. The city council shall establish fees for permits required by this ordinance. <br /> <br />1. Fees shall be due and payable at the time of permit issuance. <br /> <br />7 <br /> <br /> <br />
